Florida Nation Basketball — Participation Waiver & Release of Liability

Effective Date: September 1, 2025, and continuing thereafter
Program Covered: All basketball workouts, training sessions, practices, games, and related events organized and operated by independent coaches affiliated with Florida Nation Basketball, regardless of location or any future change in program name.

Assumption of Risk

I, the undersigned, acknowledge and understand that participation in basketball activities involves inherent risks of physical injury, including but not limited to collisions, falls, strains, sprains, fractures, or other serious injury. I knowingly and voluntarily assume full responsibility for these and all risks, whether known or unknown.

Release of Liability

In consideration of being allowed to participate, I hereby release, waive, discharge, and hold harmless the coaches, organizers, volunteers, facility owners, staff, affiliates, and any associated entities (“Released Parties”) from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to participation in the program, whether caused by negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

Medical Certification

I certify that I/my child is physically fit to participate in basketball activities and that I have adequate medical and health insurance to cover any injuries that may be sustained. I authorize program staff, coaches, or designated volunteers to seek and secure medical treatment in case of emergency, and I accept financial responsibility for such care.

Media Release

I grant permission to Florida Nation Basketball and its representatives to use, reproduce, and/or distribute photographs, video, or other media recordings of me/my child taken during program activities for promotional, educational, or informational purposes without compensation or further approval.

Parent/Guardian Consent (For Minor Participants)

By signing below, I affirm that:

• I am the parent or legal guardian of the minor named in this form.
• I have read and fully understand this Waiver and Release of Liability.
• I voluntarily consent to my child’s participation and accept the inherent risks involved.
• I acknowledge that by signing electronically or physically, I am waiving certain legal rights, including the right to sue for injuries or damages arising from participation.
• I confirm I have the legal authority to sign this waiver on behalf of my child.
