I, the undersigned, acknowledge that I/my child am voluntarily participating in
basketball workouts, training sessions, and related events beginning on __/__/__ and continuing thereafter, organized and run by independent coaches.
This waiver applies to all such activities, regardless of location or any future change in the name of the program.
I understand and acknowledge that participation in basketball activities involves inherent risks, including but not limited to physical injury, and I voluntarily assume all such risks. I further agree to abide by all rules, regulations, and instructions provided by the organizers, coaches, and facility staff.
In consideration of being allowed to participate, I hereby release, discharge, and hold harmless the coaches, organizers, facility owners, and any associated volunteers from any and all liability, claims, demands, or causes of action arising from participation in any covered event, whether caused by negligence or otherwise.
I certify that I/my child am physically fit to participate and have adequate health insurance to cover any injuries sustained.
I also grant permission to the organizers to use, reproduce, and/or distribute photographs, videos, and other media recordings of me/my child participating in the program for promotional or informational purposes without compensation or further approval.

I understand that by signing electronically, I am giving up certain legal rights, including the right to sue for injuries or damages arising out of inherent risks of participation.
